PARENT/GUARDIAN CONSENT FORM FOR USE OF AUDIO-VISUAL MATERIAL
English-Speaking Union Chile (ESU) recopilará material audiovisual para eventos patrocinados por ESU, como talleres y concursos basados en el idioma inglés. Los representantes locales de ESU solicitan permiso para utilizar dicho material para efectos educativos y de difusión de la misma actividad.
The English-Speaking Union Chile (ESU) will collect audio-visual material for ESU sponsored events such as workshops and competitions in the English language. ESU local representatives’ requests permission to use such material for educational and dissemination effects of the same activity.
COMO SE USARÁ EL MATERIAL AUDIOVISUAL:
HOW THE AUDIO-VISUAL MATERIAL WILL BE USED:
El material podrá ser utilizado en publicaciones, en nuestro sitio web oficial o en material promocional relacionado con nuestra organización y sus eventos de oratoria en idioma inglés, por nuestras oficinas tanto en Chile como en el exterior. Dado que la ESU es una organización internacional, la promoción de nuestras actividades puede implicar la distribución de nuestras publicaciones y material promocional a nivel mundial.
Para niños menores de 18 años, un padre o tutor debe completar y firmar el formulario a continuación mediante nuestra plataforma de firma digital Control Doc o manualmente. Los formularios de consentimiento firmados manualmente deben ser devueltos a esu@esu.cl en formato PDF.
Los participantes mayores de 18 años pueden firmar el formulario de consentimiento siguiendo los mismos procedimientos mencionados anteriormente.
ESU Chile no se hace responsable por el uso que terceros hagan del material audiovisual que utilizamos.
The material may be used in publications, on our official website or with promotional material related to our organisation and its English-language oracy events, and by our offices both in Chile and abroad. As the ESU is an international organization, promoting our activities may involve distribution of our publications and promotional material globally.
For children under 18 years of age, a parent or guardian must complete and sign the form below by our Control Doc digital signature platform or manually. Manually signed consent forms must be returned to us at esu@esu.cl in PDF format.
Participants that are 18 years of age or older can sign the consent form themselves following the same procedures mentioned above.
The ESU Chile shall not be liable for any third-party use of the audio-visual material we use.

Padre / tutor legal: He leído la declaración anterior y doy mi consentimiento para el uso del registro audiovisual de mi hijo / hija, generado durante una competencia, evento o actividad de idioma inglés de ESU Chile.
Parent / legal guardian: I have read the above statement and give my consent for the use of my son / daughter's audio-visual record, generated during an ESU Chile English-Language competition, event or activity.
